Support Coordination

What is Support Coordination?

Support coordination is an NDIS-funded service that helps you navigate and connect with the disability support system to achieve your goals. Think of your support coordinator as your personal guide through the NDIS journey – someone who understands both the system and your individual needs.

At its core, support coordination helps you:

  • Understand your NDIS plan and available funding
  • Connect with the right service providers and community supports
  • Build your capacity to coordinate your own supports over time
  • Navigate challenges and resolve issues as they arise
  • Prepare for plan reviews to ensure ongoing appropriate funding

Three Levels of Support Coordination

3 levels of support coordination can be included in a participant’s plan:

Support Connection (Level 1) helps you build connections with community and funded supports,

Support Coordination (Level 2) provides ongoing coordination assistance, and

Specialist Support Coordination (Level 3) focuses on reducing complexity in the participant’s support environment and helping overcome immediate and/or significant barriers.

Our qualified coordinators across Brisbane, Perth, and Melbourne deliver all levels based on your plan.

How Support Coordination works

We make the process simple and personalised:

  • Understanding your needs: We start with a friendly conversation to learn about your goals, challenges, and current supports.
  • Creating your plan: Together, we identify the right mix of services and providers to match your NDIS plan.
  • Connecting you to services: We arrange support, manage bookings, and coordinate with providers.

Ongoing support & reviews: We check in regularly, monitor progress, and help you prepare for NDIS plan reviews.

What makes support coordination different from other NDIS supports?

Unlike direct care services, support coordination is about building your independence and confidence in managing your own supports. Your coordinator works with you, not for you, helping you develop the skills and connections needed to live the life you want.

At Centre Disability Support, our qualified coordinators across Brisbane, Perth, and Melbourne focus on empowering you to achieve your goals while ensuring you get maximum value from your NDIS plan. We’re not just connecting you to services, we’re helping you build a sustainable support network that includes both funded NDIS services and natural community connections.

Experience coordination that meets NDIS Quality Commission requirements

Support coordination and plan management are ‘intermediary’ supports in the NDIS that should support a participant to work towards their goals and participate more fully in the community. Our coordinators follow NDIS Quality and Safeguards Commission standards, maintain detailed progress records, and use evidence-based approaches to coordinate your supports effectively while building your independence and confidence.

Who should consider support coordination?

Support Coordination is ideal if you:

  • Are new to the NDIS and unsure where to start.
  • Have multiple or complex support needs.
  • Need help building independence and self-management skills.
  • Are going through a life change, such as moving house or leaving hospital.

Whether you need short-term guidance or ongoing assistance, our team can make navigating the NDIS easier and help you get the most from your plan.
